Passport Invitation Questionnaire

Please complete the following stationery questionnaire. This information will be used to create your first draft. If you have any questions, please call our studio at 312-877-5077 (the quickest way to get a hold of us) or email Savannah. Please do not put questions in this form, it should be for  your final text only. Review all of your details carefully for spelling and grammatical errors, then submit!

Guest and Return Address Envelope Printing is available for $0.50 per invitation*. If you would like to take advantage of this upgrade, make sure to download the guest address template below and enter your guest names and address in the Excel spreadsheet. You can send these via email later when they are ready for print and production.

* Address printing is not available on certain dark envelopes as we cannot print in white ink/toner; contact the shop for a list of non-printable colors

    Please Note: We suggest purchasing about 5 more invitation suites than you actually need in case you forgot to add someone to your guest list. If you need to order more invitations after your original invitation order has shipped, you will be charged a $25
    ​set-up fee in addition to the cost of your invitations. So be on the safe side and order a few more. We're sure your mother-in-law will have forgotten to invite someone like her great aunt!
    This is for the font and design colors printed on the passport and RSVP. These are typically your wedding colors; please try to choose at least 2 if possible. Please note that the design and font must be printed in a matte color, not shimmer. Feel free to send example swatch colors for a better reference.

    Some examples to include are: Welcome Party, Rehearsal Dinner, Ceremony, Cocktail Hour, Reception, and Farewell Brunch. Make sure you list exact days, times, and locations of each event.

    Many of the pictures and information used to describe the resort or venue will come from the information on your wedding website (if applicable), Google, and the hotel/resort's website. If you have specific pictures and information you would like used please create a separate word document and upload them using the links below. You can also send them to [email protected].

    • This completed form
    • Photo of the Couple (for the Inside Front Cover Page)
    • Map of Country (for the Inside Back Cover Page. This is only necessary if you have a specific one you want to use; if you don't, we'll create one for you that looks similar in style to the one from the sample.)
    • Any other information and/or photos you would like used
